Identifying Health Priorities and Aligning Care

Patient Priorities Care is an age-friendly, systematic approach for identifying patients’ health priorities and aligning decision-making and care to those priorities. Our training resources are free for anyone to use. CME/MOC credits are available through the American College of Physicians (ACP) modules and the Talent Management System for U.S. Department of Veterans Affairs staff.

Our virtual workshop series provides credentialed healthcare professionals the space to practice this approach through role play, discussion and case-based coaching. Registration is accepted on a rolling basis.

This ACP module prepares clinicians to translate patient priorities into health care decisions in collaboration with patients.
