This self-directed website guides people (and their caregivers if they desire) through the health priorities identification process. At the end, they will have a template/summary that outlines their Health Priorities (the life and health goals/activities they want their health care to help them with given what they are willing and able to do) and the One Thing that they most want to focus on.

This guides the patient (and caregiver) when working with the member of the healthcare team to identify and communicate the patient’s health priorities, which includes their core life values, health outcome goals and healthcare preferences.
- Patient Summary of Health Priorities
This is for patients to provide clinicians a summary of health priorities and Specific Ask – the “one thing” in their healthcare and treatments that they would like to focus on so that they can do more of a desired activity. Patients are encouraged to brings this summary to all clinician visits.
