The more you and your healthcare team know about what matters most to you, the better you can work together to line up decisions about your health with your health priorities. Below are tools to help you get started.
This approach is meant to be a collaboration between you/your caregivers and your clinicians – we also have resources for your clinicians to support them in providing health priorities-aligned care.
