Inviting Patients to Patient Priorities Care
Introduce patients to priorities aligned care and explain why you are asking them to identify their health priorities, especially if you’ve been working with a patient for a long time.

Identifying Patients’ Health Priorities
Patients’ Health Priorities can be identified by the help of any trained member of the healthcare team or self-directed by the patient (with a care partner if desired). At the end of the process the patient should have a completed Health Priorities Template to share with their health care team and/or family. This will be used to learn more about what matters most to the patient and guide decision-making.
