Health Professional Toolkit

Patient Priorities Care (PPC) is an evidence-based approach that provides a systematic way to incorporate patients’ health priorities into decision making. We have developed a suite of tools that supports patient priorities aligned decision-making.

If you are new to the PPC approach, we recommend reviewing these resources first.

Inviting Patients to Patient Priorities Care

Introduce patients to priorities aligned care and explain why you are asking them to identify their health priorities, especially if you’ve been working with a patient for a long time.

Identifying Patients’ Health Priorities

Patients’ Health Priorities can be identified by the help of any trained member of the healthcare team or self-directed by the patient (with a care partner if desired). At the end of the process the patient should have a completed Health Priorities Template to share with their health care team and/or family. This will be used to learn more about what matters most to the patient and guide decision-making.
